EBD Services for Elderly, Blind & Disabled Individuals

Virtue Care’s EBD services provide long-term supports so adults who are elderly, blind, or physically disabled can live at home or in the community instead of a nursing facility. While EBD does not mirror the DD waiver’s 24/7 residential habilitation, it does fund community-based residential options and robust in-home supports.

What Virtue Care's EBD services provide:

Alternative Care Facilities (ACFs): assisted-living residences with 24-hour protective oversight and help with daily activities in a community setting

Home modifications & accessibility adaptations: ramps, grab bars, doorway widening, and other safety/independence upgrades (lifetime-capped benefit)

In-home supports: IHSS (participant-directed), Personal Care, and Homemaker services for daily living tasks in one's own home

Transition support: help moving from institutional care into a community home

Complementary support: Non-Medical Transportation, Respite, and community engagement as authorized in the service plan

We partner with each person and family to align services with home/community living goals, prioritize accessibility and meaningful routines, respect age, mobility, and sensory needs, and coordinate across ACF, IHSS/PC/Homemaker, transportation, and coaching so independence and quality of life stay central.
